There is increasing evidence that the etiology of MS is heterogeneous.
MS is often described as a "disease waiting to happen".
The pathogenesis of the critical transition to clinical disease according to diagnostic criteria is largely unknown.
The role of environment and chance must also be considered.
Implications for counselling "at risk" individuals, based on family history, are discussed.
